I understand that I need to consult a professional mental or other healthcare provider with any questions or concerns regarding mental health and all medical conditions.
I understand and agree to never delay or abstain from seeking medical attention, disregard medical advice, or discontinue treatment because of information provided on arcadialifecoaching.com or during coaching services provided by Arcadia Life Coaching.

I understand and agree that under no circumstances can I hold Arcadia Life Coaching/arcadialifecoaching.com, its owner or any employees liable for any actions that I take.
I understand that Arcadia Life Coaching will protect my information as confidential unless I state otherwise in writing. If I report child, elder abuse or neglect or threaten to harm myself or someone else, I understand that necessary actions will be taken and my confidentiality agreement limited in this capacity. Furthermore, if Arcadia Life Coaching is ordered by a court to provide information or to testify, it will do so to the extent the law requires.
I understand that the use of technology is not always secure and I accept the risks of confidentiality in the use of email, text, phone, Zoom, Skype and other technology. I hereby release, waive, acquit and forever discharge Arcadia Life Coaching, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by Arcadia Life Coaching as a result of the advice given by Arcadia Life Coaching or otherwise resulting from the coaching relationship contemplated by this agreement.
I agree to disclose details of any past or present psychological or psychiatric treatment. I understand and agree that Arcadia Life Coaching does not engage in therapy with coaching clients. In entering into the coaching relationship, and signing this agreement, I agree that if any mental health difficulties arise during the course of the coaching relationship, I will immediately notify Arcadia Life Coaching and seek assistance from a professional mental health or medical health provider.
I agree not to hold Arcadia Life Coaching/arcadialifecoaching.com, its owner and/or employees liable for any loss or cost incurred by me, or any person related or associated with me, as a result of materials or techniques, or coaching, offered by Arcadia Life Coaching/arcadialifecoaching.com, its owner and any employees.
